I agree to this Djinda Dreaming Vacation Care booking for the School Holidays, including payment of the days booked. I understand if my booking is cancelled within a rostered week, I will still be charged for those bookings
Excursion Authority
Excursions are an extension of the Djinda Dreaming Vacation Care program and will be child orientated. We are required to notify Parents/Guardians about any excursions including where and at what times the excursion will take place, this information is found on the Vacation Care Program attached. We leave Premises between 8.30 am and 9am and return to Premises between 3-3.30pm. A Risk Assessment plan will have been conducted on each excursion; this will be readily available to all parents on request and at the kiosk to view. We follow the ratio of 1:10. We are licensed to take up to 38 children on an excursion.

Changing Your Booking

I understand that I am required to give 24 hours notice in writing to change my booking and that this authority will be kept on file with this permission form, acknowledging permission for that excursion.
Bus Travel
I give my child permission to travel on the Djinda bus to and from Djinda Child Care Centre and the excursion destination or emergency situations. My child will be in a child care restraint suitable for their height and weight as per law.
Photography Consent
I give permission for my photographs of my child/children to be posted on social media or used in other marketing.
